Health Exam Requirements For the 2011 - 2012 School Year

SPORTS PHYSICALS

Are required prior to participation in
the first day of practice in all sports.

SCHOOL PHYSICALS
Are Mandatory for Students Entering:

Pre-K
Early Childhood
Kindergarten
Sixth Grade
Ninth Grade

School Physicals are required to be on file the first day of
student attendance in the fall. (August)

***Physicals that are tuned in over the summer are to be
placed in the RED DROP BOX located in the foyer
outside the library.
PLEASE DO NOT TURN THEM INTO THE
GRADE SCHOOL, HIGH SCHOOL, OR DISTRICT OFFICE

Sports Physicals are required prior to participating in all sports. Sport must be less than one year old and remain current throughout the entire season. Sports Physicals for fall sports that begin before school starts must be turned in 1 week prior to the first practice.

DENTAL EXAMS

are Mandatory for Students Entering:

Kindergarten
Second Grade
Sixth Grade

VISION EXAMINATIONS

Are required for students entering
Kindergarten
The deadline to turn in Vision Exam is
October 15, 2011

LEAD SCREENING
Is a requirement for:
Pre K
Early Childhood
Kindergarten

IMMUNIZATIONS

Are required to be up to date and current at all times for all grade levels including Pre-K and Early Childhood.

Immunizations are required to be up to date and current for all grade levels including Pre-K and Early Childhood.

Attendance at Pawnee School District #11 is subject to compliance with immunization and physical exam requirements set forth in the
Illinois School Code 105 ILCS 5/27-8.1

All Health Records must be on file with the
School Nurse by the first student attendance day.
