Pawnee Athletic "Physical" Information

Pawnee Athletic
"Physical" Information
Get a Physical Form Here
Sport physicals remain current for exactly one year.
Athletes must have current sports physicals on file
with the school prior to participating in
any school sport.
This includes fall sports
which may begin practicing
in the summer before school starts.
Physicals can be dropped off during the summer
in the red drop box located just outside the
nurse's office at Pawnee Schools.
Please do not turn them into the grade school or high school office.  Parents are encouraged to use the drop box location
and turn them in early so they can be reviewed
periodically over the summer.  Also, this allows
for any incomplete information to be corrected
before the first practice.

Athletes will not be allowed to practice until
the physical has been reviewed and considered
complete with the athlete's name, physician's
signature, the date of the physical, and indication
by the physician that the athlete can begin practice.

Any missing information must be corrected
before the athlete can begin practice.
Students who turn in physicals at the last minute
run the risk of missing valuable practice time.
Parents are also encouraged to make
and keep a copy of the physical
before turning it in to the school.
Many other sports organizations,
such as JFL, may not be affiliated with
the school, but also require sports physicals.
By law, the school is not permitted to
share medical information about our student-athletes.
